All sections of the form need to be filled.

The form has to be signed - so you need to download and sign it before sending back to us.

We cannot complete the enrolment process without a form of ID. Please send us a copy of your ID together with the enrolment form. Passport or Birth Cert is best!

 

You can email it. Scan it. Bring it into the  Grey Lynn Community Centre reception. Post it to The Good Medicine Clinic, 510 Richmond Rd , Grey Lynn 1021.

 

We will organize the transfer of medical records from your previous clinic once we are open - you do not need to do this.

We are happy to post forms for those who prefer this or you are welcome to pick up a physical form from reception during our opening hours.
